Completion of your client questionnaire may take up to two hours. Please get comfortable and set aside some quiet time to focus on the questionnaire. It is important that every question be answered, that all assets be listed, and that all creditors be listed with their complete mailing address. The mailing address should be the address indicated on two of the last three statements you have received from the creditor.
